All treatment centers within the Guardian Recovery Network work with most major insurance providers, including Humana. Humana’s diverse line of health insurance plans include benefits for what Humana health insurance providers refers to as SUD, or Substance Use Disorders. Benefits extend to medication assisted treatment, individual and group counseling, and inpatient treatment. SUD treatment is not available on all plans. In addition to SUD treatment, Human’s Behavioral Health benefits extend to coverage for co-occurring disorders such as depression, anxiety, bi-polar disorder and more. Members may find out if their plan includes behavioral health benefits by dialing the membership number on the back of their Humana plan identification card.

While Humana operates in all 50 states, each Humana plan will vary based on your state of residence, and your specific plan. Deductibles, co-pay amounts and network providers will be different for each plan. Individual plans purchased through the Health Insurance Marketplace are categorized into tiers of bronze, silver, gold and platinum. While bronze plans typically cover 60% of medical costs after payment of a high deductible, platinum plans cover approximately 90% of costs with a lower deductible, but higher monthly payment.

Every health plan’s benefits will vary. A range of deductibles, copayments and coinsurance rates are found across a variety of Health Maintenance Organization, or HMO plans and Preferred Provider Organization, or PPO plans. In addition, Humana may offer the lesser known Exclusive Provider Organization (EPO) plan or High Deductible Health Plans (HDHP) in your state. Not all Humana health plans include behavioral health coverage. Only plans purchased through the Health Insurance Marketplace are required to do so by law.
